Choosing Between Insurance and Private Pay for Your Private Practice: What You Need to Know

When starting a private practice, one of the most frequently asked questions is whether to accept insurance or operate on a private pay basis. This is a crucial decision with many factors to consider, and it’s perfectly okay to make adjustments as you go along. Remember, you don’t have to get everything perfect from the start. Sometimes, you might need to change your approach as the needs of your practice and clients evolve. Here are some key considerations to help guide your decision:

The most important factor is to choose a payment model that suits your needs and preferences. Many practice owners choose their payment model based on client demands, but this can lead to burnout and dissatisfaction. With dedication and effective marketing, it is certainly possible. For example, I run a group practice with over 15 therapists that operates on a cash-pay basis. While it requires effort, it is achievable.

Insurance-based practices typically need to negotiate rates with insurance companies. These rates can vary, with some insurance companies offering favorable rates and others less so. As a result, insurance-based practices often need to see more clients to achieve the same income as a private pay practice, which may see fewer clients but charge higher fees. Building a private pay practice can be more challenging in rural areas. In such communities, you might need to consider accepting insurance to attract clients or adjust your rates to remain competitive.
